Global Service Center – Request for Service Form

All instruments sent in for service must have an assigned return authorization (RA) number & signed proof of instrument decontamination form. Instruments received without assigned RA and signed decontamination form will be returned without service. All calibrations are ISO 9001-2015/NIST compliant and include before and after data (ARD) where applicable and 2-day courtesy returned shipping within North America.

Customers outside of North American must provide freight collect account information to return your instrument once serviced.

Please allow 24-48 hours to receive your RA and decontamination form.

EVALUATION FEE FOR REPAIR -  ALL PRODUCTS – [$630] 
A deposit is required to reserve an evaluation. Once evaluation is complete, an estimate of actual repair costs will be emailed for approval. Approved estimates will require submission of PO or credit card. If repair estimate is rejected, the evaluation fee will be invoiced and the instrument returned as is. Services are invoiced after the maintenance is complete and the instrument has been returned.
